Personal Care Services (PCS) provides in-home aides to assist with personal care needs that are directly linked to a medical condition.
Services are available by referral from physicians, nurse practitioners, physician assistants, or hospital discharge planners to Carolina's Center for Medical Excellence (CCME). A patient must have seen his/her primary physician within the last 90 days to qualify for a referral.
The referral forms may be found at
A home assessment is performed by CCME to determine if the patient qualifies for services. CCME then establishes the number of in-home aide hours per month and daily tasks to be performed.
The patient is given a choice of providers. If Yancey County PCS is selected, a nurse visits the patient's home to develop a plan of care. A nurse will visit at least every three months to ensure the services continue to meet the patient needs.
PCS are covered by Medicaid. PCS can also be covered by the VA, Vocational Rehab, and some individual insurance policies.
